If you as a driver are asked to comment on an incident by the press/media what must you do?

- Do not make any comment

- Refer them to the media relations officer

If you were riding on a train involved in a train accident, list 3 things you would be allowed to access the cab to help deal with the incident.

- Secure the cab

- Get protection equipment

- Use the GSM-R

- Make note of short life evidence

- Carry out rescue operation

If you did have to enter the cab in the above situation what must you take note of?

- The original position of things

- Any short life evidence

Following a fatality on the line where recognisable body parts would only be visible to the driver of a passing train. We can run trains providing what two things?

- Driver is made aware of the circumstances

- Driver is happy to do so

What 3 things would make a piece of equipment defective?

- Isolated

- Missing

- Not performing its intended function either fully or partly

Name 2 times a train can enter service with defective equipment in the cab

- Cab is boxed in

- If the route you are going over is not fitted with that piece of equipment

- If it is allowed for in the DOTE contingency plan for the purposes of movement for repair

What are the four things you must do if have defective equipment which will affect the onward movement of the train?

- Stop Immediately and contact the signaller

- Stay where you are unless otherwise instructed

- Await instructions

- Act on instructions

From the signallers point of view if you have stopped at an unsuitable location, what would they do?

Give you permission to move to a more suitable location to await instructions

Which pieces of defective equipment require a competent person to travel with the driver if available

AWS/TPWS

DVE/DSD

Broken/obscured windscreen

Specify who is a competent person to travel with you for the above defective equipment

Another competent person such as a Driver, Driver Manager, Driver Trainer or Driver Instructor. For AWS/TPWS and Broken/obscured windscreen the driver must sign the route so they are aware of what is coming up. For DVE/DSD they must have the required skills/rule knowledge to carry out the instruction or procedure if required.

What are the following persons duties when moving a train with defective TPWS in order to get the train out of service?

The Drivers duties on approach to signals

The Drivers duties on approach to speed restrictions

The Competent persons duties on approach to signals.

The Competent persons duties on approach to speed restrictions

The Drivers duties on approach to signals - Provide running commentary calling out signal aspects and indications and speed reduction on approach to a cautionary or stop aspect.

The Drivers duties on approach to speed restrictions - Provide running commentary calling out the speeds of any warning boards or change in speed restrictions and explain you are applying the brakes to adhere to the speed restriction.

The Competent persons duties on approach to signals - Acknowledge the driver's reaction to approaching signals and where necessary remind the driver of any approaching signals.

The Competent persons duties on approach to speed restrictions - Acknowledge the driver's reaction to speed restrictions and remind the driver of any speed restrictions ahead where necessary.

If you were the competent person in one of the above scenarios and the safety of the train was about to be compromised, what would you do?

Stop the train immediately by applying the emergency brakes and discuss with the driver why you have taken this action.

List at least 4 options SDC have regarding what to do with a train with defective on train equipment in service

- Take the train out of service

- Send a fitter out

- Box it in

- Take it to a safe place to be examined by a driver

- Send a replacement train or traction unit

-

What must you do if you have a train with defective on train equipment which will not affect the onward movement of your train whilst in service?

- Advise the SDC at the earliest opportunity and act on their instructions

- Advise the signaller if there will be any delays caused

When a train has operated a hot axle box detector, how will you be advised of this fact by the signaller and how will you be advised regarding which axle box has set it of?

The signaller will change the signals to bring you to a stop by using the normal signalling sequence.

The signaller will advise you which side the hot axle box is on and how many axle boxes from the front of the train it is.

How would you check the axle box concerned and how would you determine if the heat is normal or not?

Place your bare hand as close to the top of the axle box as possible to feel the heat coming off the axle box. Compare the heat with other axle boxes to see if it is excessively hot.

If the heat from the hot axle box was normal, which other axle boxes would you check in this scenario

You would check all the axle boxes on the same coach and if need be you would also check all the axle boxes on the coaches either side.

What are the speeds restrictions for the following defective equipment if you are taking the train out of service?

DVE defective, AWS/TPWS ok, no competent person

Confirmed hot axle box (2 speeds)

Headlight out

Headlight out and portable headlight attached - 75mph

Obscured windscreen and no competent person - A speed you feel that you can safely control the train (no set speed)

Horn partially defective - Linespeed

Horn fully defective - 20mph

TPWS defective, competent person provided and foggy conditions - 40mph

After operating a 2nd hot axle box - 20mph

Defective speedo - Safe speed which will not be exceeding any speed restrictions

If you have operated the EBS on a double unit, what should be done with your passenger if possible and why?

You must if possible move the passengers in to the front section of the train in case the train divides. This is because the EBS has been operated so if the train was to accidently divide then the brakes would not automatically apply on your section of the train so you might not be immediately aware of the incident. Ideally you would leave a competent person in the rear section if one if available.

You have operated the TIS and need to move the train to detrain the passengers. What must you do with the doors before starting?

What must you do with the TIS when changing ends?

Manually check all the doors on both sides of the train before setting off to ensure they are all safely shut.

When changing ends you must reset the TIS where possible and reinstate it at the opposite cab you will now be driving from.

Describe two ways you could perform a rotational wheel test

- Get someone to observe the wheel while you move the train

- If by yourself then mark the wheel and rail with a piece of ballast and then move the train a little before checking the markings to see if the wheel has rotated.

If a cab secure radio area goes down then when must you be accompanied by another person with a mobile phone?

At all times if on a DOO passenger train

Who is the first person you must contact if your train is stopped because of a train fault?

The signaller

What is the main purpose of the Cut and Run policy?

To reduce the impact to other services by making a decision with in the allotted time regarding whether the train will be fixed or whether an assisting train will be needed.

During peak periods the time in the previous question will be reduced so who must you contact for help?

The fitters

During fault finding as soon as you realise that your train will be unable to continue, what must you do?

Contact the signaller and arrange for an assisting train

If you are required to look on both sides of your train on a running line to identify a fault on a defective unit then what must you do?

Contact the signaller and get the lines either side of your train blocked before you leave the train.

When you are requesting an assisting train list 3 things you would tell the signaller

- Nature of the problem

- The exact location of the failed train

- Give the signaller assurance that you will not move your train

- Agree the direction which assistance will come from

- Any additional information the assisting driver may need to know

You have requested assistance and you are walking through the train to get to the rear to lay protection when you realise you can fix the problem. Can you fix the problem and proceed?

Only if you have the signaller's permission

If you have requested assistance what would you check when walking past the rear or front of your train to lay protection?

That your train has the correct lights showing

You have failed and requested assistance and the signaller tells you that assistance will come from the front. Identify the assistance you will lay and where you will wait for the assistance to arrive.

Lay Assistance Protection by placing 3 detonators, 300m in front of your train and 20m apart. You will wait with a red flag a head of the furthest away detonator.

You have failed and have requested assistance and the signaller tells you that assistance will come from the rear. Identify and explain the two options for protection that you could lay and where you would wait in the following...

You have a signal in 200m rear of your train

Signal in 200m rear of your train - Walk to the signal and lay the protection and then contact the signaller at the SPT. If the signaller confirms that the signal is providing the adequate protection then wait there displaying a red flag otherwise you must move the protection to the full 300m and wait there with the red flag.

You have failed and have requested assistance and the signaller tells you that assistance will come from the front. Identify where you will wait and where you will lay protection when the 300m will fall in the middle of a tunnel.

Lay the protection the normal 300m in the tunnel and continue to walk in the direction the assistance will be coming from until you are out of the tunnel and wait there with the red flag.

You have failed under normal working arrangements but are unable to contact the signaller from your train. Identify what you would do and the protection you would lay. Level crossing 400m away from the front of the train and behind the train 3/4 mile is a signal.

Walk in the direction of crossing a lay assistance protection at 300m before carrying on to the crossing to use the telephone to contact the signaller.

If you have laid assistance protection in one direction and the signaller has now advised you that assistance will now come from the other direction, what would you do?

Agree with the signaller the expected time it will take to return to the new protection point. Pick up the detonators already laid and then lay assistance protection 300m from the train in the other direction.

Your train has failed and you require assistance during TBW but you are unable to contact the signaller from your train. Identify the protection you would lay and how you would contact the signaller

Lay assistance protection 300m to the rear of the train and continue 1.25m to the rear of your train to lay emergency protection. Carry on walking until you reach a form of communication and then make contact with the signaller. Rule book also shows TCOPs.

Your train has failed during single line working and you request assistance and the signaller tells you that assistance will come from the rear. Identify what protection you will lay and where you will wait for assistance to arrive

Assistance protection 300m away in the rear (3 detonators, 20m apart, red flad)

Name 4 things you would need to know from the signaller if you were asked to go in to a section to assist a failed train.

- Exact Location of the failed train

- What the problem with the failed train is

- Where the driver of the failed train will be

- How the failed train is protected

- Whether you need to pass a signal at danger

- Where the train will continue to

- Permission to make a unsignalled wrong direction move

At what speed would you enter the section to assist a failed train?

Proceed with caution being able to stop short of the failed train.

You have gone in to the section to assist the failed train, what must you do if you come to a tunnel and you have not picked up the failed train's driver?

Do not enter the tunnel unless you can be certain the driver is not in the tunnel and this may involve walking the tunnel to confirm this. Stop immediately if you explode the first detonator and wait for the driver to come to you.

Explain the risk involved if you are the assisting train which stops at a signal on approach to a failed train and stop to pick up the driver of the failed train.

You could pick the driver and become distracted and then continue to proceed without gaining the signaller's permission to pass the signal at danger.

When will you not be required to lay any protection if your train has failed and needs assistance?

When failed on a permissive line and the assisting train will come from a signalled direction.

What is contained in the following fire extinguishers

Red/Beige

Red/Black

Red/Blue

Red/White

Red/Beige - Foam (Aqueous Film Forming Foam)

Red/Black - CO2

Red/Blue - Dry Powder

Red/White - Water

What are the restrictions on using an AFFF extinguisher?

Must not be used on electrical voltages which is higher than 110v

What two things must you do immediately you come to a stand to visually warn oncoming trains that your train has just been involved in a train accident?

- Red handsignal

- Sounding train in distress warning with the warning horn

- Hazard warning indication

List four things that are classed as a train accident?

- Collision with a person

- Collision with another train or rail vehicle

- Collision with a vehicle at a level crossing

- Collision with an obstruction which might affect other running lines

- A fire on a train which might endanger other trains passing it

- A fire on a train which might mean passengers are detrained

- Derailment

If involved in a train accident what are your four main priorities?

- Stopping of trains on obstructed lines

- Protect of trains on obstructed lines

- Gaining communication to request emergency services and advise of the incident

- Safety of the Passengers

If the pass com is used and you have the ability to override it, you must only use it to get to where?

A suitable location to deal with the incident

When must you never override the passcom?

- When leaving a station

- If you think the train is in danger

When the emergency brake is applied without you doing anything what three things would you immediately check for?

- Check TPWS brake demand light

- Look out the window to check if the train has divided

- Look for any illuminated bill lights

Apart from displaying a hand danger signal, list 3 other ways you could stop an oncoming train in an emergency

- GSM-R emergency button

- Carry out emergency protection

- Hazard warning indication

- Sounding train in distress warning with the warning horn

1. Your train is involved in an accident which is blocking an adjacent line, you gain contact with the signaller and request emergency service and the signaller gives immediate assurances he can protect the running line. What is the minimal protection you must lay down before dealing with your passengers.

Place TCOPs on the running line affected.

You are laying emergency protection and the normal distance falls with in a tunnel, explain how it will be laid.

Place a TCOP on the affected line. Walk 1.25 miles and then carry on until you reach the end of the tunnel before placing the 3 detonators at 20m apart from each other. Wait at the furthest detonator with a red flag.

You are laying emergency protection and the normal distance falls after a diverging junction. Explain how it will be laid.

Place a TCOP on the affected line. Walk 1.25 miles and then carry on until you reach the diverging junction and then decide which direction to prioritise and continue in that direction for the full 1.25 miles. Placing the 3 detonators at 20m apart from each other. Wait at the furthest detonator with a red flag unless you need to place further protection at the other line of the diverging junction.

What are the two other times you would be required lay three detonators down before you reach 1.25 miles when laying emergency protection?

- If you see a train approaching

- If you reach a form of communication

When would you need to lay emergency protection in rear of your train on the line on which you are standing during the train accident scenario?

During TBW when you have no contact with the signaller and if you are unsure if your train is operating the track circuit block

If you had other obstructed lines to protect during your accident where would the protection of the line on which your train is standing come in order of your priorities?

Last

During a train accident you’re on your own and you have emergency protection to lay on lines in both directions, explain how you will decide which way to protect first. Give at least 3 things

- On which line a train is most likely to approach

- What might happen if the line is not protected

- how well the driver of an approaching train will be able to see the obstruction

- How badly each line is obstructed

- Linespeed of the obstructed lines

What are the first three things you will do if you become aware that your train is on fire while working under AC OLE?

Drop the pantograph

Stop the train

Inform the signaller

You have stopped because of a small train fire and you are extinguishing it as you notice people have started to evacuate the train onto the track, what is your priority now?

Inform the signaller immediately and get the running lines blocked and CRE switched off before attempting to extinguishing the fire agin.

You have extinguished a small fire on your train and are able to continue, what would you do in the affected coach?

Move passengers into the most forward carriages which are unaffected and if possible leave a competent person with a fire extinguisher in the affected coach in case the fire starts up again.

In a train accident, where is the safest place to leave your passengers if you can?

On the train

In order to carry out any train evacuation, what three things must you ensure is carried out first?

- Lines have been blocked

- The third rail has been isolated

- The signaler has been informed

List all the things you would consider in an emergency evacuation to ensure things go as smoothly as possible in the circumstances, before, during and after the evacuation?

- Ask the signaller to block the affected lines

- Obtain third rail isolation/possible OLE

- Place a TCOP on each affected line

- If the signaller can assure you all the affected lines are blocked then you can go a head with the evacuation. Otherwise carry out emergency protection on all obstructed lines and contact the signaller before carrying out the evacuation.

- Seek assistance from other competent persons

- Decide the where on the train and which side is the safest to evacuate from

- Decide the easiest way for passengers to leave the affected vehicles

- Decide where you will take the passengers

- Use the PA system to tell passengers what to do

- Tell passengers to stay in a safe place once evacuated and appoint a competent person to keep them there

- If safe to do so conduct a sweep of the train

- Remain with the passengers in a safe place and meet the emergency services when they arrive

You experience an emergency brake application and look back and find that you once had 8 coaches and now you only have 4 and you cannot see the rear 4 coaches. What must you presume has happened to the rear four coaches and what would you immediately do about it?

You must stop immediately and assume the rear 4 coaches have derailed and is blocking lines. You must contact the signaller immediately to arrange blocking of all lines which could be affected and lay any protection required.

Who would you need to talk to to gain permission to recouple a train that has divided in service?

The signaller and Rolling Stock Technician

If you cannot recouple a divided train and you are going to leave the rear portion in the section what protection would you lay?

Assistance protection either side of the divided portion which you are leaving because you are not currently aware which direction assistance will be coming from?

What must you place on the rear of the front portion before proceeding?

Tail lights

What would you say to the signaller if you spot a dangerous goods train accident and what specific information should you give them about the train involved if you can?

'This is a dangerous goods emergency'

- The emergency code of the affected vehicles and other details from the hazard warning panel

1. What would you do if you exploded one or more detonators in the following circumstances?

In the middle of nowhere with no hand signaller about?

At a signal box or where a red hand signal is displayed?

In the middle of nowhere with no hand signaller about - Stop and try to contact the signaller to find out what is going on. If unable to contact the signaller you should proceed with extreme caution towards a possible obstruction.

At a signal box or where a red hand signal is displayed - Stop immediately and do not move until given permission to do so.
