Treatment of complicated UTI/Pyelenephritis with no risk of MDR organism
-Treat with Flouro for 7 days and Bactrim for 14 days as outpatient
-Treat for 14 days if inpatient
Ceftriaxone 1 g IV daily
OR
Zosyn 3.375 or 4 g IV q6h
Levofloxacin 750 mg IV daily
Levofloxaxcin 750 mg PO daily
Ciprofloaxin 400 mg IV BID
Ciprofloaxcin 500 mg PO BID
Ciprofloaxin 1000 mg PO daily
If pyelonphenritis with risk of MDRO
Imipenim 500 mg
Meropenem 1 g q8h
Dorepenem 500 mg
If there is suspicion of gram positive infection superimposed with gram negative uti. Add?
Vancomycin 15 mg/kg q12
Linezolid 600 mg IV or PO q12
Daptomycin 6 mg/kg q24 hr
