Hypertrophic Cardiomyopathy
Calcium Channel Blockers:
Verapamil (Calan) CCB of choice. Decreases ventricular contractility and improves myocardial relaxation reducing ventricular wall tension resulting in increased ventricular filling.
DO NOT USE: positive inotropes. Dioxin (Lanoxin), dobutamine (Dobutrex), dopamine (Intropin), milirinone (Primacar), epinephrine & norepinephrine (Levophed).
Anitdysrhythmics : Amiodarone (Cordarone) useful for ventricular or atrial dysrhythmias in both obstructive and non-obstructive hypertrophic cardiomyopathy.
