Tropical pancreatitis.
Pathophysiology
Intraductal plugging & obstruction
Direct toxins & metabolites - act on pancreatic acinar cells to stimulate cytokine release —> stimulate stellate cells to produce collagen & fibrous tissue.
Cytokines also stimulate inflammation by neutrophils, macrophages & lymphocytes.
Oxidative stress - eg. Idiopathic pancreatitis
Necrosis-fibrosis - recurrent acute pancreatitis healing with fibrosis
Ischaemia - from obstruction & fibrosis
Treatment
Stop offending habit (etoh)
Analgesia - involve CPS
Steatorrhea - creon
Diabetes - insulin
Still not working —> surgery (Puestow/Frey)
