Granuloma

localised aggregation of modified macrophages.

focal area of granulomatous inflammation.

Epithelioid cells

Activated macrophages.

Finely granular, pale eosinophilic cytoplasm and central, ovoid nucleus, which is less dense than that of a lymphocyte.

Granulation Tissue

Tissue of repair.

TB

Basic pathology = granulomatous inflammation + caseous necrosis

Histology of granulomas

T-lymphocytes and plasma cells on the periphery.

Plasma cells have clock-face pattern of nuclear chromatin.

Histology of Epitheloid cells

Eosinophilic cytoplasm, poorly defined cell borders and elongated nuclei.

Langhans cell

Multi-nucleated giant cell, formed when materials digested by histiocytes cannot be degraded.

Represent fusion of macrophages -- non-phagocytic.

Histology of Langhans cells

Large cells, with peripheral nuclei.

Nuclei form wither a u-shape or complete circle at the periphery of the cell.

Ghon Complex

local spread in primary pulmonary TB infection

subpleural granulomata, 1-2cm in size, at base of upper lobe or apex of lower lobe.
