Communication is key because we must listen to the client and communicate with the nursing team our comments or concerns to be able to provide adequate care to the client

Evaluation is measuring the effectiveness of assessing, diagnosing, planning and implementing. The client is the focus of the evaluation.
State at least three methods of evaluation.
Client: the primary source of evaluation criteria is the client
Team Conference: a conference is helpful not only to plan nursing care, but to evaluate the effectiveness
Community Health Agencies is another way of evaluating outcomes of care by reaching out to these agencies for follow up information about the client
Discuss the statement: Discharge planning begins upon admission of the client into a facility.
The client and family cannot be expected to remember a large amount of teaching at one time, especially just as the client is leaving the facility.
