alprazolam (Xanax)

Benzodiazepine, anxiolytic



Indications: generalized anxiety disorder, short term relief of anxiety, panic disorders, anxiety associated with depression



Usual dose: orally, no more than 4 mg/day



Adverse effects: confusion, ataxia, headache



Contraindications: narrow angle glaucoma, pregnancy



Interactions: alcohol, oral contraceptives



Administration: oral and tablet

diazepam (Valium)

Benzodiazepine, anxiolytic



Indications: anxiety, management of alcohol withdrawal, reversal of status epilepticus, preoperative sedation, adjunct relief of skeletal muscle spasms



Usual dose: 2-10 mg 2-4 times/day



Adverse effects: headache, confusion, slurred speech



Contraindications: hepatic dysfunction



Interactions: alcohol, oral contraceptives



Administration: oral, rectal, injectable

lorazapam (Ativan)

Benzodiazepine, anxiolytic



Indications: agitated patients going in mechanical ventilation, alcohol withdrawal, panic disorders



Usual dose: 2-6 mg/day divided in 2-3 doses



Adverse effects: confusion, ataxia, headaches



Contraindications: pregnancy, narrow angle glaucoma



Administration: can be given IV, but is irritating to muscle and must be diluted

amitryptyline (Evavil, Endep)

TCA



Indications: was depression, but now insomnia and neuropathic pain



Usual dose: 10-300 mg/day



Adverse effects: dry mouth, constipation, blurred vision, urinary retention, dysthymias



Contraindications; pregnancy, recent heart attack



Interactions: anticholinergics, MAOIs, carbamazepine



Administration: very potent anticholinergic effects

doxepin (Sinequan)

Antidepressant, anxiolytic



Indications: depression, anxiety



Usual dose: 75-150 mg/day



Adverse effects: anticholinergic, drowsiness, ataxia, tardive dyskinesia, tremors



Contraindications: suicidal thoughts, glaucoma, children under 12



Interactions: drugs metabolized by P450 2D6, MAOIs, cimetidine, tolazamide

nortriptyline (Aventyl, Pamelor)

TCA



Indications: depression, mood problems



Usual dose: orally, 1-4 times daily



Adverse effects: drowsiness, dizziness, anticholinergic, weight gain, trouble urinating



Contraindications: glaucoma, liver problems, recent heart attack, pregnancy



Interactions: blood thinners, anticholinergics


selegiline (Ensam)

MAO-B inhibitor



Indications: major depression



Usual dose: 6mg/24 hours



Adverse effects: back pain, hypotension, weight gain, impulse control disorders



Contraindications: meperidine, allergy



Interactions: tyramine containing foods, meperidine



Administration: this refers to the patch only

trazodone (Desyrel)

2nd generation, triazolopyridine



Indications: depression, insomnia



Usual dose: 25-600mg/day



Adverse effects: strong sedative qualities



Contraindications: allergy



Interactions: alcohol, CNS depressants



Administration: can impair cognitive function in older adults; has been associated in rare cases with transient non sexual priapism

bupropion (Wellbutrin)

2nd generation



Indications: depression, aid in smoking cessation



Usual dose: 209-450mg/day bid



Adverse effects: dizziness, confusion, tachycardia, agitation, tremor, dry mouth



Contraindications: allergy, seizure disorder, MAOIs



Interactions: azole, antifungals, CNS depressants



Administration: oral form only

citalipram (Celexa)

SSRIs



Indications: depression, OCD



Dose: by mouth, short half life



Adverse effects: anxiety, dizziness, drowsiness, insomnia



Contraindications: allergy, MAOIs, certain antipsychotics



Interactions: highly protein bound drugs, St. John’s wort



Administration: commonly associated with discontinuation syndrome


duloxetine (Cymbalta)

SSNRI



Indications: depression, generalized anxiety disorder, diabetic peripheral neuropathy, fibromyalgia



Dose: 20-40mg/day or 60mg/day bud



Adverse effects: dizziness, drowsiness, headache, GI upset, anorexia, hepatoxicity



Contraindications: allergy, concurrent MAOI use, glaucoma



Interactions: SSRIs, triptans, alcohol



Administration: available only for oral use


fluoxetine (Prozac)

SSRI



Indications: depression, bulimia, OCD, panic disorder, premenstural dysphoric disorder



Dose: 10-20mg/day, but up to 80 mg/day



Adverse effects: anxiety, dizziness, drowsiness, insomnia



Contraindications: allergy, concurrent MAOI therapy



Interactions: MAOIs, benzodiazepines, blood thinners



Administration: considered the prototypical SSRI

haloperidol (Haldol)

Antipsychotics



Indications: long term treatment of psychosis, nausea



Dose: 0.5-5.5mg bid-tid PO, IM, IV



Adverse effects: extraprymidal symptoms, amenorrhea, weight gain, rash, tardive dyskinesia



Contraindications: hypersensitivity, Parkinson’s, depressants



Interactions: CNS depressants, grapefruit juice



Administration: manufacturer states not to give thru IV but has been done safely before

chlorpromazine (Thorazine)

Antipsychotics



Indications: hallucinations, schizophrenia, bipolar disorder, nausea, hiccups, tetanus, anxiety



Dose: 2-4 times a day



Adverse effects: EPS, lightheadedness, insomnia, weight gain



Contraindications: Parkinson’s, glaucoma



Interactions: anticholinergic, dopamine increasing drugs, seizure activity



Administration: do not use in older adults with dementia

clozapine (Clozaril)

Atypical antipsychotics



Indications: psychotic illness (schizophrenia)



Dose: no more than 900 mg/day



Adverse effects: agranulotosis, sedation, increased appetite, EPS, tardive dyskinesia



Contraindications: allergy, myloproliferative disorders



Interactions: alcohol, CNS depressants, antihypertensives



Administration: patients beginning this require weekly monitoring of WBC count for 6 months


risperidone (Risperdal)

Atypical antipsychotics



Indications: schizophrenia



Dose: 1-6 mg/day



Adverse effects: EPS, tardive dyskinesia, sedation, anticholinergic



Contraindications: allergy



Interactions: CNS depressants, alcohol, antihypertensives



Administration: oral and injectable form

ziprasidone (Geodon)

Antipsychotics



Indications: schizophrenia and bipolar disorder



Usual dose: by mouth



Adverse effects: EPS, seizures, blistering, unusual bleeding



Contraindications: allergy



Interactions: blood pressure meds, diuretics, levodopa, carbamazepine
