1. Intrinsic Factor Pathophysiology?
a. Glycoprotein secreted in the stomach that binds to B12.
b. The intrinsic factor-B12 complex then attaches to receptors in the distal ILEUM and is absorbed.

a. These children exhibit worsening irritability, loss of appetite, and decreased activity between ages 1-5.
b. They are at risk for permanent neurologic damage resulting from spinal cord demyelinization.
8. Tx of Juvenile Pernicious anaemia?
a. IM B12.
9. Other causes of macrocytic anaemia?
a. Diphyllobothrium latum, which causes B12 deficiency.
10. What are children who are exclusively fed on Goat’s milk at risk for?
1. Deficiencies of B12 and folate
2. Brucellosis if the milk is unpasteurized.
11. Where is intrinsic factor produced?
a. Gastric antrum.
12. What should be considered in pts w/macrocytosis but normal B12 and folate levels?
a. Atypical bone marrow pathology such as leukemia or myelodysplasia).
b. Referral to a paediatric haematologist would be warranted.
13. What other CBC abnormality is periodically seen in pts w/B12 deficiency?
a. Thrombocytopenia. Thought to be related to impaired DNA synthesis and infective thrombopoiesis.
