What is cardiogenic shock?

Failure of the heart pump resulting in decreased cardiac output and thus decreased tissue/organ perfusion

What is the most common cause of cardiogenic shock?

Myocardial infarction

What are the symptoms of cardiogenic shock?

- pulmonary oedema (dyspnoea, crackle sound in lungs)


- peripheral oedema (distended jugular vein oedema)


- hypotension


- chest pain


- cool, clammy skin


- oliguria

What is the main underlying issue in cardiogenic shock?

Decreased contractility of the heart muscle

What medications could you give in cardiogenic shock?

- diuretics (if pulmonary/peripheral oedema)


- vasopressors which cause vasoconstriction


- dopamine


- dobutamine

What type of drug is dopamine and dobutamine?

Positive inotropics

What do positive inotropic medications do?

Increase heart contractility

What is a side effect of dobutamine?

Worsening hypotension because it causes vasodilation
