Density of masses
Fat and mixed density masses are benign

Fat containing masses
-oil cyst
-lipoma
-galactocele (fat-fluid level)

Mixed density masses
-hamartoma or fibroadenolipoma
-lymph node

Fibrograndular density - high, low, and isodense
Mass shape
Round
Oval
Lobular
Irregular (suspicious)
Mass margin
Circumscribed
-need to see >75% margin
Microlobulated
Obscured
Indistinct (ill-defined)
Spiculated
Management of masses
A mass may be followed up (BIRADS 3) at baseline exam if:
Shape: round, oval, lobular
Margin: circumscribed

Mass - additional views
Benign features? (contains fat or skin lesion)
No - U/S
Cyst or lymph nodes
No - Round, oval, lobular? Circumbscribed?
Yes - BIRADS 3
No - BIRADS 4/5
Asymmetry
Area of fibroglandular tissue that is visible on one view only.

80% represent superimposition of normal breast tissue.
Global asymmetry
-Substantially greater volume of fibroglandular in one breast than in the contralateral breast.

-Occupies more that one quadrant.

-No assoc. mass,calcifications,or architecture distortion.

-almost always represents a normal variant
Mass vs. Focal asymmetry
Both
-space occupying lesion, seen in 2 projections

Mass
-convex-outward contour
-denser in center than at periphery

Focal asymmetry
-concave-outward contour
-usually interspersed with fat
-if no assoc. findings, <1% likelihood of malignancy
Focal asymmetry work-up
-repeat the same view
-change beam obliquity slightly
-change breast obliquity (roll view)
-use spot-compression mag technique
-targeted u/s

if no mammographic, sonographic, or palpable abnormality:

Baseline: BIRADS 3

Stability of at least 2-3 years: BIRADS 2.
Developing asymmetry
Focal asymmetry that is new, larger, or denser than the prior study.

BIRADS 4
Breast asymmetries
With some exceptions:
Asymmetry BIRADS 1
Global asymmetry BIRADS 2
Focal asymmetry BIRADS 3
Developing asymmetry BIRADS 4
