Barbituates - Physicochemical
Differ based on C5 substitutions
1) Thiobarbituates (thiopental) 2) Oxybarbituates (methohexital) Basic pH --> when mixed with acidic drugs (neuromuscular bx) they cause precipitates that block lines |
Barbituates - Pharmacokinetics
Mostly undergo hepatic metabolism to inactive metabolites that are extreted through urine and conjugated bile.
Phenobarbital is excreted unchanged through the urine |
Barbituates - Pharmacodynamics
MOA - enhancement of inhibitory neurotransmission (GABAa) and inhibition of excitatory transmission (unclear)
Rapid onset - unconsciousness in less than 30 sec Has long context sensitive half time (prolonged recovery with continuous infusion) CNS - dose dependent sedation. No analgesia. Potent vasoconstrictors --> Dec CBF, CBV, and ICP (used for intra cranial masses) CVS - mod dec in BP due to peripheral vasodilation + dec symp response RES - depressant via dec TV and RR leading to dec MV. Doesn't dec airway reflexes as well as propofol t/f NEED NEURO BLOCK. |
