What fluid do you use when administering blood products?
-Normal Saline ONLY!!
-->no dextrose soln' or lactated ringers
-->no other additives should be given via the same tubing

-during the 1st 15mins or 50ml the RN should remain with the pt

-rate should be no more than 2mL/min

What steps should the RN take when an acute blood reaction occurs?
1. stop the transfusion
2. maintain a patent IV line c NS
3. notify the blood bank & HCP immediately
4. recheck ID tags & #'s
5. monitor VS & urine output
6. tx ssx per MD order
7. save the blood bag & tubing & send them to blood bank for exam
8. complete transfusion reaction reports
9. collect required blood & urine specimens at intervals stipulated by hospital policy to evaluate for hemolysis
10. document on transfusion reaction
What time frame do acute reactions occur after a blood transfusion starts?
-15 mins
What time frame do delayed reactions occur after a blood transfusion ends?
-2-14 days after administration
What should the RN do if a clt has an acute hemolytic reaction after a blood transfusion?
-tx shock if present
-draw blood samples
-maintain BP c IV colloid soln
-give diuretics to maintain urine flow
-insert indwelling cath or measure amounts of hourly urine output
-do not transfuse aditional RBCs
What should the RN do if a clt has a febrile reaction after a blood transfusion?
-give antipyretics as perscribed
-do not restart transfusion
What should the RN do if a clt has a mild allergic reaction after a blood transfusion?
-give antihistamines as directed
-if ssx are mild & transient, transfusion may be restarted
What should the RN do if a clt has an anaphylactic & severe allergic reaction after a blood transfusion?
-initiate CPR if indicated
-have epi ready for injection 0.4 mL of 1:1000 soln SQ or 0.1ml 1:1000 sol diluted to 10ml c saline for IV use
-do not restart transfusion
