Independent in daily cares, finances, decision-making skills. Can learn new information.
Allen Cognitive Level 5
Independent in daily cares, assistance may be needed with finances, decision making, and
organizational skills. Subtle problems with memory may be noted. Can learn new information.
Allen Cognitive Level 4
Physically independent with daily cares, assistance needed to initiate/monitor quality of cares.
Increased assistance needed with all cognitive skills. Notable problems with memory are noted. New
information can be retained only after much repetition. Transitional stage for supervision. By the end of this level, most individuals’ will require supervision to assure safety and assistance to complete cares with quality.
Allen Cognitive Level 3
Assistance with all daily cares is needed. Full assistance required with all cognitive skills
including initiating, sequencing, judgment, problem solving, and decision making skills. Significant memory impairment is noted. Unable to learn new information. Difficulty with language skills are observed.
Allen Cognitive Level 2
Dependent in cares. Significant impairment of all cognitive skills. Loss of language skills.
Decrease in motor and visual/perceptual skills. No longer recognize familiar people or objects.
Allen Cognitive Level 1
Totally dependent in all cares. Generally bed ridden. All basic needs met by caregiver.
