Why Screen?

-Exercise can increase the risk of sudden cardiac death and heart attack


-Cardiovascular events usually only occur in clients with preexisting condistions


-Many clients are unaware of their risk

Purposes of Screening


Client Screening should identify:

-Medical contraindications to exercise


-Need for medical evaluation and exercise


-Clinically significant disease and the need for medically supervised exercise


-Other special needs

Purposes of Screening

-The Personal Trainer is in the best position to evaluate the client's risk factors or medical conditions

Risk Factor: Age

Men >= to 45 yr



Women >= to 55 yr

Risk Factor: Family History

-Myocardial infarction


-Coronary revascularization


-Sudden death before 55 yr in father or other male 1st degree relative


-Sudden death before 65 yr in mother or other female 1st degree relative

Risk Factor: Cigarette Smoking

-current cigarette smoker


-those who quit within the previous 6 months


-or exposure to environmental tobacco smoke

Risk Factor: Sedentary Lifestyle

-Not participating in at least 30 min of moderate intensity physical activity (40% - <60% VO2R) on least 3 days of the week for at least 3 months

Risk Factor: Obesity

-Body mass index >= to 30 kg/m^-2


-or waist girth >102 cm (40 in) for men


- >88 cm (35 in) for women

Risk Factor: Hypertension

-Systolic blood pressure >= to140mm Hg


- Diastolic >=to 90 mm Hg


-Confirmed by measurements on at least two separate occasions


-or on antihypertensive medication

Risk Factor: Dyslipidemia

- Low density lipoprotein cholesterol >= to 130 mg/dL (3.37 mmol/L)


-High density lipoprotein cholestrol <40 mg/dL (1.04 mmol/L)


-On lipid lowering medication


-If total serum choleterol is all that is available, use >= to 200 mg/dL (5.18 mmol/L)

Risk Factor: Prediabetes

-Impaired fasting glucose= fasting plasma glucose >= to 100 mg/dL (5.55mmol/L) and <=to 125 mg/dL (6.94 mmol/L)



-Impaired glucose tolerance = 2 h values in oral glucose tolerance test >=to 140 mg/dL (7.77 mmol/L) and <=199 mg/dL (11.04 mmol/L) confirmed by measurements on at least two separate occasions

Negative Risk Factor: High-density lipoprotein cholesterol

>= to 60 mg/dL (1.55mmol/L)

The Screening Process: Signs and Symptoms ( Major signs or symptoms suggestive of cardiovascular, pulmonary, or metabolic disease)

-Pain, discomfort (or other anginal equivalen) in the chest, neck, jaw, arms, or other areas that might result from ischemia



-Shortness of breath at rest or with mild exertion



-Dizziness or syncope



- Palpitations or tachycardia



-Intermittent claudication



- Known heart murmur



- Unusual fatigue or shortness of breath with usual activities


-Orthopnea or paroxysmal nocturnal dyspnea



-Ankle edema

Clearance/ Referral: Medical clearance or referral:

- Obtain medical clearance if indicated



-Complete informed consent



-Conduct appropriate assessments



-Refer to physician or other health care provider if warranted
