Salter-Harris classification
I - fracture through epiphysis only, excellent prognosis
II - fracture through part of epiphysis and metaphysism, commonest, excelent prognosis III - intra articular fracture into epiphysis, accuratre reduction needed but good prognosis IV - intra articular fracture across epiphysis into metaphysis, need reduction, resonable prognosis V - crush injury to epiphysis, subtle findingm poor prgnosis |
Guustilo and Anderson open fracture classification
Grade 1 - small clean wound < 1cm by tip of fracture
Grade II - >1 cm with mod. soft tissue injury and mod contaminated Grade III A - large lac, debridement wound <10cm Grade III B - >10cm, crushed tissue and contamination Grade III C - vascular injury Grade IV - total amputation |
Principles of fractures & dislocation Mx.
Anaglesic
Reduction (immediate ass with exposed bone or vascular compromise) Temporary immobilisation Stabilisation (casting, fixation) Follow up (plaster check, rept Xray, Xray report) Rehab |
Donway Splint
Lower limb splint/ pneumatic tractuion device
Sole of foot applied to a solid metal plate then strapped to it via a stirrup bicycle pump mechanism extends the splint to provide traction need good anaglesic and femoral nerve block |
Options of leg splint:
Air splint
donway splint Hare Traction splint Thomas splint MAST suit (suitable for pelvic/ lower lmb) |
Advantages and Disadvantage of synthetic casts (fibreglass):
Adv:
- lighter, moist resistant, stronger, less messy, more radiolucent than plaster Disadv: - not water proof, keep in airtight enviroment, increase pressure under cast from initial post fracture swelling, more expensive |
Which of these fracture should not use a bord arm sling?
clavicular fractures AC inhury Shoulder dislocation Slbow fracture Forearm fracture fracture shaft of humerus |
fracture shaft of humerus - should use clooar and cuff
(also for supracondylar fracture) |
