A request for Admin Sick must be made during this time frame

At least 2 hours before start of tour

If the sickness you were given Admin sick for continues into the next day, you may request a 2nd Admin sick in this manner

Call the desk at least 2 hours before start of tour. The 1st and 2nd Admin sicks will count as 1 sick incident

S - Supervisor determines inappropriate


L - Late, called it in with less than 2hrs


L - LOD Injury or Illness (hurt/sick on job)


I - Injuries (hurt off job)


C - Chronic B (you go sick way too much)


M - Major Illness (flu, pneumonia, etc)


D - Dental Problems


+ - Stress Related/Psychological

When reporting sick (regular or admin), the UMOS must do this

Notify the DO at least 2 hr before start of tour by phone, in person or by competent messenger




Have a responsible person do it, if you're too sick to call yourself, still at least 2 hrs before tour





If a MOS is reporting sick and their command is closed, or if they are ineligible for/refuse Admin sick, what shall they do?

Notify Medical Division Sick Desk direct


Get Serial # & provide it to DO


Advise Sick Desk member of any address or phone # changes


Get the name of the Sick Desk member & record time notified


Stay at residence/other authorized location unless permission to leave is given by District Surgeon or Sick Desk Supervisor

When the DO receives a request to report sick, they will first offer this in cases of minor, non-LOD illness, if eligible and advise them of these restrictions

Offer them Admin sick, if their eligible




Advise them that each admin sick is for 1 tour & they have to stay home or at authorized location unless permission to leave is given by District Surgeon or Sick Desk Supervisor

If MOS goes admin sick, the DO will indicate it here

On pertinent records (Roll Call, Sick Log, etc), if MOS accepted admin sick

What will the DO do if UMOS is ineligible or refuses admin sick?

DO will direct UMOS to go see the Dept Surgeon

Once the UMOS calls the DO and advises them of the sick serial # and the location where they are authorized to remain, the DO will do this

Call back to verify that the MOS is really at the stated location

The DO will notify the Sick Desk Supervisor in these circumstances

If MOS is not at stated location




If special or unusual circumstances exist regarding member reporting sick (reporting sick from out of state or country)

If the MOS is suspected of malingering, the DO notifies this unit

Medical Division's Absence Control & Investigations Unit

In addition to recording pertinent information (including sick serial #) in the Sick Log, the DO will also do this

Make appropriate entries on pertinent Roll Calls & notify relieving DO/Supervisor if MOS is scheduled to perform duty with either of the next 2 platoons

Since information regarding a MOS going sick is to be treated in a confidential manner, who has access to the Sick Log and other sick records?

Only to the persons authorized by the CO for the proper discharge of their duties

How does the DO/supervisor make sure that the MOS who reported sick is listed as being on sick report?

By checking Medical Division Sick Reporting System via Dept Intranet

The DO/supervisor must check the Medical Division Sick Reporting System at the start of tour. List the reasons why they must check it @ the start of tour

To find out who is on sick report and/or not on full duty status

If MOS is already on admin sick and can't report for duty because they are still sick, who do they notify?

DO/supervisor at assigned command & Sick Desk at least 2 hours before start of tour. They will continue to be on sick report and report to Dept Surgeon. It will count as 1 sick incident, upon consolidation by the Medical Division

At the next office hours, after reporting sick, the UMOS must report to Dept Surgeon. If UMOS is unable to visit the Dept Surgeon, who must they notify that they can't make it?

UMOS on terminal leave may not be placed on sick report unless this happens

Unless sickness or injury is the result of LOD action while on terminal leave

Who designates an "old LOD injury"?

Dept Surgeon, upon examination & interview of the requesting UMOS. UMOS will not be permitted to report sick with a self-designated diagnosis of "old LOD injury"
