CHIEF COMPLAINT: Progress note for speech therapy and feeding therapy.

Patient continues to come to ambulatory care once a week for his feeding and speech therapy. He has participated in five speech therapy session since his initial evaluation.

Status of Goals:
1. Patient will correctly demonstrate a yawn/sigh to reduce tension in the vocal mechanisms during 5/10 trials.
2. Patient will all response to let the air out slowly when imitating: A. Phrase level utterance. B. Imitating sentence level response. C. Spontaneously producing a sentence level response. D. During conversational exchange with 80% accuracy. Status: A. Using a carrier phrase such as A+ and label. He is 50-90% accuracy with maximum support. B. Imitating at the sentence level 50%. C. Patient will begin vocalizing at the onset of exhalation using, A. Vowels, B. Labeling objects using the phrase A+ a noun, C. By producing sentences or answering questions that require just a one sentence response with 80% accuracy. Status: A. With vowels 90% accuracy, goal completed. B. Labeling objects utilizing phase A+ noun 50-90% accuracy with maximum assistance. Goal continued. C. By producing sentences or answering questions that require just a one sentence response. Status: 50% accuracy.
4. The patient will recall functional information following a 3-5 minute delay with less than or equal to two cues in order to increase functional integration into his environment. Status: This goal has not been addressed, ongoing.
5. Patient will demonstrate use of 3-5 memory strategies with minimum support during 3-5 tasks. Status: Therapist has introduced two strategies such as word chaining and work association but he has not mastered these strategies. Goal
