In addition, organizations are encouraged to tailor healthcare to the needs of each individual patient; and patients and families are encouraged to participate and be involved in their own care
Re-designation of Primary Care Re-designation of primary health involves expanding the basic care provider spectrum to include a variety of professions to better care for complex patients needing a multidisciplinary team involvement. Furthermore developing shared plans of care, better coordination between primary care and hospitals, making scheduling easier and more community resources available (McCarhty & Klein, 2010).

Controlling Healthcare Costs The rapid increase in health care cost has become a major concern for the US. One of the major concerns of the Triple Aim is to increase “value for the money”. The initiative ensures a better allocation of resources by: assuring that reimbursements and resources are furnished to services that support the goals of the Triple Aim, forming partnerships with physician groups and health care organizations committed to reducing health care cost, and rewarding those who produce a better and more efficient healthcare (Institute for Health Improvement, 2015).
