I would recommend that you meet with her privately in the conference room at the end of the day (rather than doing it earlier on, since she’d then probably feel self-conscious the whole rest of the day). You have to also consider emotional issues. As you know this is a very sensitive topic so be very polite. Finally, prepare yourself mentally; writing down what you are going to say will help tremendously.

“Phillip during the convey positive intent step, you have to briefly state what you would like to discuss and why. Remember you motivation is to make an improvement and not to blame Sally. I would suggest saying something along these lines to her, “Sally thank you for taking the time to meet with me. You are a very valuable team member. I would like to discuss our dress code/personal hygiene policy because as you know each employee is a representative of the organization and lately you have not been meeting the dress code expectations.”

Link her lack of personal grooming to the impact it has not only her coworkers but also the business. You can state one or two consequences that can happen if she does not make any changes. I would suggest telling her the following, “Sally, you are a representative of the company. When you do not present a well-groomed appearance you are portraying a negative impression of not only yourself, but also the company. Having an unpleasant odor makes it hard for others to work around you. As you can see personal hygiene is very important.
Ask the other person to respond
“The fifth step is allowing Sally to respond back. Listen to what she has to say without interrupting. Phillip you can ask her open-ended questions to better understand the situation such as, “What is you view of this situation?” or “Is there anything that you want to me?” Also remember to keep the focus on the topic and be open to receive feedback from Sally.
