My colleagues expressed concern really on what is going on in the health care system especially about continuity of care. Many patients they have encountered have been concerned about seeing different doctors in the clinics. Because our hospital is a teaching hospital, it is hard to have the resident doctors seeing the patient all the time. The hospital have very few attending doctors. One of the colleagues recollected a story of old man who was in his early seventies admitted with hypertensive crisis because he did not take medications. He told her that the doctor prescribed medication which he reacted from before because he was not familiar with him and I guess did not look up well in his chart. He didn’t know so he went to pharmacy to get it but he was told he had a reaction to it before so he needs a new prescription. It was a Friday and the clinics are closed weekend. So he had to wait for Monday. He cannot stay three days without medication so his blood pressure went up and he had to call 911.The colleague expressed that this is unnecessary admission in which it can be avoided if the same doctor was seeing the patient. Also they pointed out that patients tend to be more comfortable if one doctor is seeing them because they is development of trust and mutual respect between them. The response from my colleagues has opened to eyes to the things I didn`t even thought of like if the Nurse Managed Clinics and Medical Home will help our Emergency Department have less patients than they should, as patients are seen at home by their PCP or just go to Nurse Managed health Clinics for minor issues. All their responses were great and just added on what I presented to them. They too did not have much of a glimpse on the ongoing health reforms. But this discussion we had, has made them feel different about what is going on in the health care system and it is
