In the cockpit, all of the instruments were destroyed and all of the levers were pushed into the cockpit panel even though the throttle quadrant was damaged in the crash. However, rescue and recovery personnel were unsure if they disturbed controls and levers, and between the fire and impact damage it was inconclusive the positions of the actuators. Tracing the flight controls cables showed that the cable had the proper integrity, but burning made actuator positions indistinguishable. This company possessed a clean FAA record with no major violations, except for “record management issues” which were minor offenses. According the maintenance company, Zonk Air was known for not writing up maintenance deficiencies until the maintenance came over and looked at the aircraft, which was the case for the fuel leak, and no squawks for maintenance deficiencies were noted for the day of the accident. By reviewing the forms, which were noted as incomplete and lacking sign-offs, it was determined several deferred maintenance actions were required, such as, repair loose cooling baffling on the right engine, tightening friction controls on the cockpit power quadrant adding cargo nets and straps to the aircraft flight kit, inspecting and replacing first aid kits, adjusting the pilot’s seat springs and locking the travel adjustment slide on the pilot’s
