Monday -Friday 8:30am -5:00pm
Daily, M-F between the hours of 8:30 am - 5 pm;Travel is required for the purpose of home visits assessments, meetings and trainings.
The activities tahta re conducted are; daily contact with agencies, participants/gaurdians; included but not limtied to inbound/outbound calls to schedule home visits/assessments. Fax is also utilized to communicate participant services

By completing an assessment of the participants needs, resources may be provided to the participant to assist in meeting the basic needs in for their lifestyle and way of living. Essentially giving the participant independence with the support received from a caregiver, allowing them to remain in the community.
I coordinate services/case management for medicaid clients, Assit participants who have spenddown Medicaid that are eligible for HCBS waiver services. Complete 40 home visits per month with participants, their family and/or guardians, friends, payee, and /or responsible parties to determine eligibility for HCBS programs/complete assessments. Complete approximately 40 assessments per month, subratcting 2 per day for State Holiday or leave used. Coordinate HCBS program services by making/receiving daily contacts with clients and providers; completing service authorizations in the HCBS WebTool. Develop and implement care plan that meets the client’s unmet needs by making client contact, collateral contacts and home visit(s), considering client choice of providers. Assure HCS service is appropriately authorized in respective data systems. Coordinate information and requests with other regional evaluation teams or assessment teams when a collaborative effort is needed. Coordinate services/case management with non-Medicaid clients when spenddown issues
