Aging people find it hard to adjust to the new life changes. Elderly individuals frequently have to find means to cope with all kinds of changes in their lives which may include social, financial, and health issues. To cope with the situation, most elderly people consider spending their old age in a nursing home, community-based center, or they employ professional social worker in their home. In this case, the most likely psychological problems to be handled include depression, anxiety, stress, and memory loss, and the most likely physical health issues to be handled include diabetes and pain management. In this case, a clinical social worker with mental health training will be involved in working privately with the family to evaluate the individual medical and social needs of the two elderly, and create and manage a treatment plan (Rosenberg, 17). Case management mostly involves trying to help elderly people at their home setting. In this case, the family is highly involved in managing the elderly and ensuring proper care. This form of care is given due to the advantage a familiar environment play in enhancing quality care. The family members get to learn more about health condition and management of their loved ones and take the main role in ensuring quality care. The social worker will also keep a clear record of regular assessment report to monitor the client’s health condition. The other service is the development and implementation of treatment plan based on the assessment results. In this case, individual treatment plan is developed based on physical, social, and psychological needs. Psychosocial therapy will highly be integrated in the treatment plan. This information will then be communicated clearly to the family members in charge of taking care of the elderly couple. The social worker will offer guidance on how the care and treatment procedure should be carried out in his/her absence.
