On this date worker visited Mr. Ules Jerry Duncan at WBMC, for the purpose of monitoring his health. When worker arrived, Mr. Duncan was sitting in his recliner eating lunch. He stated he was participating in therapy and would do small exercises while lying in bed. He stated he did not want to go to the NH and would like to go back to Laurel Gardens. Worker told him Ms. Jones was willing to except him back if he could walk and take care of himself. While there, Mr. Duncan asked if hospice would come out and see him at Ms. Jones, worker recommended home health instead so he could continue physical therapy at
