REASON
CHIEF COMPLAINT: Right forearm lesion.
BLANKLINE
The patient was referred for a right forearm lesion that he claims has since resolved. He gives a history of a skin cancer of unknown type that was excised from his left chest about 10 years ago. He has questions concerning several stuck on keratoses over his trunk and extremities that he claims are similar to the resolved site on his right forearm. He also has a 3-4 month history of an asymptomatic slowly enlarging site of his right second dorsal finger. He is also using triamcinolone 0.1% ointment p.r.n. scaling in his ears and scalp.
PHYSICAL EXAMINATION
He has mild to moderate scaling of seborrheic dermatitis bilaterally in his external auditory canals, and mild scaling in
