Section 5 outlines transition services and activities. These are a set of activities for the student that are designed to focus on improving the academic and functional achievement. Transition services should be based on the results of transition assessments. Services and activities should be listed that will be needed to support the student’s measurable postsecondary goals. The IEP team needs to consider if the student needs services or activities for each category. This determination is based on formal and information assessments. A begin and end date should be provided for each service or activity. The anticipated duration of the service or activity should be provided. This could be one time per week, four times a month, single occurrence,
