At Apple Hills Pharmacy medication incidents are handled according to the OCP guidelines. We follow the dispensing error protocol posted online and the template form available. Although we try to be 100% accurate, sometimes medication incidents are a reality. They always must be handled by the pharmacist. It should be deal with it promptly by being honest to the patient that the dispensing error occurred. If the drug was ingested we have to establish the risk to the patient. The Poison Contact center should be contacted to guide us through the process. If the risk is high, the patient should be referred to ER/ hospital or a physician. If the risk is low then, we have to assure the patient that the drug would likely not cause any side effects
