Samuel Seium. The help that I would seek from my supervisor would be everything because I want to gather the most information that I can for myself and the betterment of this client. I would have a list of goals that I would show to my supervisor and get recommendations on what I should add or take away. When goals are picked, I would make a list of interventions and seek guidance from my supervisor on what should be added or removed. I would have the same formula when developing outcome measures so we can create realistic measures within ten sessions.…
When I started working as a CAN, as well as volunteering in the health care field, I understood that the work involved included helping residents in activities of daily living. What I found out after I started working was how much of an impact the job makes on people. When I was volunteering to help a woman go grocery shopping I did not think much of it. However, when I came into her home and started putting her groceries away she started tearing up. I found out that she and her husband used to always go grocery shopping together and would later play a game that involved putting the groceries away.…

Diagnosis The client, Gwen Cummings, comes to the inpatient substance use treatment facility presenting with apparent symptoms of severe substance use disorder and potentially childhood trauma. According to the criteria from the DSM-5 on Substance Use Disorder, the “substance is often taken in larger amounts or over a longer period than was intended” (American Psychiatric Association, 2013). Gwen shared that she and her boyfriend recently woke up late prior to their attendance at their friend’s wedding, unable to remember the details leading up to the end of the night. This, and other shared accounts from the client indicate that she met the first criteria for substance use disorder.…
Mr. James is back for this third session where we will be reviewing the goals that we tentatively discussed during the last session and to finalize them in order to move forward with our tasks. We will determine if the goals that we recently talked about are sufficient or if we need to add more in order to paint the big picture to the Judge. The more classes that are taken, the better the chance of reunification; however, if provider reports do not suggest that there is a behavior change within the client, the tasks will be obsolete. There must be a behavior changed identified within the client for the department to recommend the reunification. Self-Determination Mr. James has the right to self-determination.…

• Explain the importance of reflective practice in continuously improving the quality of service provided. The plan should list each objective or goal, the services needed to reach that goal, and methods to follow up. Once the plan is complete, the helper should start to arrange the provision of services for the client. Also, if the helper is planning on utilizing a strength-based approach in the planning phase, a good understanding of the client’s strength is essential to effectively plan for their services. An ethical consideration to be cautious about in this phase is to not push or force the client in any situation that they don’t want to be in. The helper should always suggest not force or tell them to do something.…
