History tells a story about a time that the hospital, LTC system and mental health were all connected to one another and were all serviced in the same manner. Well, maybe there wasn’t really a hospital, nursing home or asylum so to speak of but there were people who had conditions or were poor and could not take care of themselves which resulted in a need which brought life to the health care facilities and models of care that we recognize today. Early in American history, few people lived to be old, but for those who did, “old-age security” meant having children or property. The public welfare system of those times was fashioned after the English “poor Laws”. Early on, paupers were given cash payments referred to as “outdoor relief”.…
Although FNPs fill an important role in our health care system, focusing my studies and practice on gerontology will allow me to better provide for the older adult population. My first semester clinical site was on a medical-surgical floor in Medical Center North, and most of the patients on that unit are older adults who were from surrounding rural counties. Throughout this rotation, my patients taught me the immense difficulty of managing chronic disease on top of working, parenting, and maintaining quality of life, especially when providers are inaccessible. Meeting with residents at Cumberland View Towers for Community Heath has re-enforced the complexity associated with primary care in older adults. As health issues increase in complexity and resources decrease, it is difficult for patients to…
Living in the United States as a woman is difficult. Living in the United States as a poor woman of color, on the other hand, is a completely different burdensome situation. Not only are women of color subject to the trifecta of sexism, racism, and classism, but they are also exploited by the government for their labor. In their essay, “Organizing Home Care,” Jennifer Klein and Eileen Boris argue that home care is the government’s method of saving money by taking advantage of poor women of color. As opposed to nursing homes, which are viewed as costly forms of institutionalization, home health care is the preferred option by both clients and the client’s loved ones, who are unable to care for them themselves.…

Healthcare Trends. In the 1980’s there was a shift toward privatization of health care as corporations began not only to integrate the hospital system, but also entered many other health-care related businesses and consolidate control (Evashwick, 2005). In 1981, the first assisted living opened in Portland, Oregon as an alternative living options for older adults who were unhappy with nursing homes. It provided a type of elderly care that offered a level of attention and independence less than a nursing homes, but more than an independent living situation. The rapid growth of this industry over the next twenty years was due to the changing attitudes of consumers who viewed the institutionalized style of nursing homes and rumors of mistreatment and neglect as no longer acceptable for most aging seniors and their families.…
Their goal is to have the majority of the population being cared for at home as opposed to in clinics and hospitals. The United Kingdom has an increasingly aging population, similar to the United States. In order to keep up with the high demand for elderly care, the government has put a large sum of money into community healthcare. This change will allow patients to remain independent and in the comfort of their own home, while also saving money by keeping patient’s out of acute care settings. Additionally, in home care allows medical professionals the opportunity to give one-to-one, personalized care.…
This growing group will have increasing education levels, including more minority elderly, and include more women than men. The most vulnerable are older single women with low incomes, baby boomers with less education, and minority groups with low economic status (Woodside & McClam, 2009). Those aged 85 and older are also at risk. Addressing issues associated with physical, social, mental, and emotional decline means providing support to aging clients and their family Woodside & McClam, 2009). There are three models that represent different orientations in service delivery: the medical model, the public health model, and the human service model.…
