Care Model Process Model

While reviewing the RWJF report, the Care Model Process really stood out to me. As a result of Health Partners hiring nurse practitioners in addition to physicians and physicians assistants, this model was developed. This model was developed to ensure quality care is delivered as a result of nurses roles transforming and expanding. This model was developed from a very popular and well-respected model called the Wagner Chronic Care Model (Robert Wood Johnson Foundation, 2012). I like how this model does address that each professionals ' role is clearly defined and this model encourages collaboration from all health care providers to ensure adequate and high-quality care is delivered to patients. I would like to share The Care Model 's policies and practices, which include, "Creating collaborative care teams, using standing orders to empower nurses and other clinicians to provide the full scope of services allowed under their licenses, providing access to routine care via the Internet, standardizing best practices" (Robert Wood Johnson Foundation, 2012, p. 3) all of these factors are very important and these are the reasons this model stood out to me when reviewing the RWJF report.
