In private practices, the primary care provider has the final decision on strategy, financial decisions, implementation of new process and overall control of how their practices run. Speaking with a Quality Manager from a local health center, Clinica Sierra Vista, I was informed that Community Health Centers are controlled by a management team and a board of directors, there may be a few physicians such as the Chief Medical Officer on the team, but aside from that there is a lack of representation from the care providers which up until recently, never been addressed. It has always been the culture of this organization to allow the management team to control and implement and providers to serve. Mississippi’s Certificate of Need (CON) process plays a central role in planning for the state’s health facilities, based on need for services, location, size and other factors (Mississippi State Department of Health, n.d.). Mississippi is one of many states to have utilized a CON program throughout the years and in some instances the program is comparable to the CON programs in other states. At the current time, Olmsted County Public Health Department (OCPHD) is working with Olmsted Community Hospital and Mayo Clinic to meet the needs of Olmsted County. The OCPHD considers five health problems in the community to be a priority. These priorities include mental health, obesity, financial stress or potential homelessness, diabetes, and vaccinations against preventable diseases. The OCPHD currently has several programs to assist the community with these priorities; however, many are not well known. Maintaining the health of one’s residents is one of the most important jobs of a government. In some state, it is too expensive to provide health care for so many people in need, yet in contrast I would argue that it is worth the cost and an investment. It is unjust for the Oregon government to choose to cut back on providing health care to its residents rather than cut back on other less vital aspects of its budget or find a way to raise more money to pay for it. The Department of Health and Human Services has created a pilot program to determine whether or not social needs such as housing, hunger, violence, transportation and more are correlated to quality and affordability in Medicare and Medicaid.
