Daphne has been a full-time employee with Vitas Health Care for a long time. Her work begins on May 20, 2004. She was employed as a hospice aide. In discussions Daphne and I have had over the previous year; she communicated her involvement in her community church activities she aids a few services in her congregation one of which is the sick and elderly in her community in which, she can present Vitas benefits in a positive light for individuals who have the requirements for hospice services.

As we approach the time of the year when Vitas take pride in giving recognition to Vitas Best, this letter is Team 302, formal way of letting Daphne know how much the team appreciates her work. The team has been impressed by your nursing aide skills, customer service skills and your ability to care for the most vulnerable population, the terminal elderly.

Along with your hard work and dedication, you have been an asset to the interdisciplinary team communicating pertinent information about your patients and families that proved very effective specifically in the area of patient care. The nurses in the facility where you cared for your patient speak very highly of your patients’ care skills and customers’ relations, they said that since they have been working with you they have seen nothing but your best.
