REASON
CHIEF COMPLAINT: Visual field defects in left eye.

BLANKLINE
Inmate was found to have visual field defects in the left eye and was sent in for evaluation. Inmate states that "all of a sudden" while walking he noticed decreased vision in his left eye and he cannot see the tip of his nose from the left side. This occurred about five years ago. The patient states three months prior to this recognition, he was hit in the head and about the face with a "lock sock." The patient states he was also kicked about the head and face. Inmate states a CAT scan was done at that time which showed a bleeding on his left side reportedly and that he was treated for a "concussion." He states he has had no change in the scotoma since that time.
