The patient is a 30-year-old female who I saw on July 21, 2015 for her physical. At that point, she was noted to have two perianal [____] lesions. These were treated with liquid nitrogen and she does tell me they fell off after several days, but now she has another one that has come. She thinks it is in a different spot.

Objective
Vital Signs
Blood pressure 110/80. Pulse is 60. Other vital signs per flow sheet.

General
Patient is alert, oriented, no acute distress.

Genitals
Perianal area does show a polypoid lesion at about the 8 o'clock position, several millimeters from her os. There is also a small lesion at the 6 o'clock position just at the anal verge.

A/P
Venereal warts. These were small and treated
