Upper Extremity Pain: A Case Study

DOI: 11/6/2013. Patient is a 40-year-old female consumer loan specialist I who sustained a work-related injury to her neck, shoulders, and arms due to repetitive work.
Per progress report dated 01/02/16, patient complained of right upper extremity pain. Current medications are Celebrex, Gabapentin, Cymbalta, and Norco. Patient has been treated with 18 sessions of physical therapy and TENS unit use. She has been prescribed with Cymbalta for pain and depression. She has completed pain coping skills and reports that this has been helpful. She is having a depressed mood and would possibly benefit from individual psychotherapy.
Based on the progress report dated 03/17 /16, the patient presents for bilateral upper extremity pain. Pain level has increased since last visit. Patient rates her pain with medications as 5/10 and without medications as 9/10.

The patient is taking her medications as prescribed. No side effects are reported. Her activity level has increased.
Patient trialed back to work for one day and reports she was unable to tolerate her work. She experienced increased bilateral left upper extremity pain and swelling to her wrists. She reports her work is computer based and cannot type and click the mouse all
