The reporting party (RP) stated residents have unexplained bruises that go unreported. The RP stated residents Silvia Dexter, Richard in room #218, Louise Minor, and Kay Hernandez have all fallen within the last few months resulting in bruises not have gone unreported. According to the RP Silvia fell last week which resulted in a bruise to her right hip. The RP stated there are insufficient caregivers to provide appropriate care to all residents. Consequently resident are left on toilets while caregivers attended to other residents which result in falls. Subsequently the staffing levels decrease on the weekend resulting lack of care. Residents are forced to stay in wet/dirty diapers for extended periods of time. The staff members who are present
