Circumstance: CSP made face to face contact with MHS and Juliet to monitor Juliet’s well-being.
Action: CSP and MHS discussed Juliets’s recent medical visits and contact with DSS worker. MHS explained Juliet’s WIC appointment was good. Juliet’s weight was not checked. The youth will continue on the same formula. MHS stated DSS worker explained no family visits are scheduled at this time. A statement from the DSS worker for the MUSC team to allow MHS to request all medical changes must be written and not verbal was not received in the mail as of date. CSP explained to MHS her responsibility of getting the correct paperwork before leaving the physician’s office. MHS explained Juliet’s is meeting all milestones. Juliet was crying for one on one
