CCIB received a complaint via phone call from RP stating that his Aunt Anna has been at this facility for 4 years and has had several UT infections the latest on 4/20/2017. RP stated that Anna was admitted to Medical Center of Modesto hospital on 4/20/2017 at 2AM, where Nurse Mickey stated that Anna had “old fecal in her vagina which is why she has had several UT infections”. RP stated that he contacted the Skyline Place Senior Living to talk to Anna’s nurse (female name unknown) about the issues with Anna, and she stated to RP “that she will have the staff do training on how to correctly wipe clients”. In addition, RP reported that Anna sister Eleanor last week on the 4/12/2017 went to visit Anna to see her in soiled bedding. RP mentioned
