URICA Questionnaires

The URICA questionnaires were downloaded and printed out for distribution. During the treatment phase called, Initial Contact, all patients were asked if they would be willing to complete questionnaires for academic research purposes (Please refer to Appendix A for recruitment script). Once the patient agreed, he or she were provided a printed copy of the questionnaire at that pre-exposure mark and on the date his or her discharge. The participants completed the questionnaires independently and handed completed copies to any staff on the unit.
