Nutritional Case Study: Type 2 Diabetes in Adulthood This case study will go over the Type 2 Diabetes in adulthood. This case study will show the issues affecting the individual, ABCD’s of nutritional assessment, and the nutritional plan of care for this patient. Mr. Giovanni Constantino is a 58 year old man with a 5 year history of Type 2 diabetes. He was previously diagnosed in 2010 and at that time he had symptoms indicating hyperglycemia for 2 years before diagnosis. His blood glucose levels during fasting indicated values of 118-127 mg/dl, which was explained to him as signs of “borderline diabetes”. The patient recalled past episodes of nocturia associated with large pasta meals and Italian pastries. At the time of his first diagnosis, he was advised to lose at least ten pounds, but no action has been taken. Mr. Constantino has been referred by his family physician to a diabetes specialty clinic. He presents with recent weight gain, limited diabetes control and foot pain. He has been trying to exercise and lose weight for the past 5 months, but has been unsuccessful. One of the medicines Mr. Constantino has been taking is glyburide at 2.5mh every morning, but has stopped taking his meds because of dizziness along with sweating and agitation. He will also need to start doing some sort of exercise regimen by at walking on a daily basis for at least 30 minutes per day. The weight loss will be an important step in lowering his blood pressure. He also needs glycemic control and was prescribed metformin at 500mg 2x per day. He has also been taught how to use a blood glucose machine and will start using this to be sure that his sugar levels stay at the proper levels and if they aren’t, he can make the necessary diet changes to be sure this does not get out of
