Tracey Case Summary

A: REFERRAL INFORMATION Tracey, a seventeen-year-old Caucasian female, was referred to a mental health social worker at Primary Care Northside by the clinic’s head physicians. Tracey, permanently separated from her biological family of 8, came to the clinic for a yearly wellness checkup. The physician ruled out all medical consideration and all of Tracey’s blood work came back positive and within a healthy range. However, since Tracey’s expressed physical symptoms, such as sadness, emptiness or feeling down, hopelessness, low self-esteem, self-criticism or feeling incapable and trouble concentrating, the physician handed Tracey’s files to the clinic’s Behavioral Heath and Social Service Team. Also, Tracey’s Foster mother informed the physician that Tracey has been avoiding peer social activities unless it is an African American boy, which the adopted family is not proponents of. According to the Foster mother, Tracey has not been productive at home or at her part-time job as she was before the feud. Also, the Foster mother noted that Tracey has anxiety when at work or outside in the community. The mother shared that she thinks Tracey is depressed and has been for a year. Per the mother, Tracey has not received any professional help for depression or the symptoms that she been experiencing. Tracey expressed interest to therapy sessions but shared hesitation about the effectiveness of family therapy because it didn't keep her biologically together.
