Toxic Arthritis Case Study

P1 had clear lung sound in all lobes and normal heart sound on auscultation; regular apical, radial, and pedal pulses; capillary refill was less than 3 minutes, +1 pitting edema in his lower extremities, skin was warm to touch. The patient exhibited poor skin turgor due to limited fluid intake. The pt. had ecchymosis on his upper extremities due to multiple IV lines insertion and erythema on his sacral area due to limited ambulation. Due to pt.’s history of acute-on-chronic renal failure, his BUN level was 59, normal levels are 7 to 20 mg/dL; Normal creatinine levels for males is 0.6 to 1.2 mg/dL, his labs showed a value of 2.69. Also, due to pt.’s diagnosis of toxic encephalopathy secondary to possibly septic arthritis, P1’s WBC count was 13,000, normal levels are 4,500 to 10,000 mcL. Apparently, because pt.’s history if arthritis, he developed nodules on his left elbow which was the reason why the pt. wasn’t able to do anything with his left arm. According to the pt., doctor aspirated accumulated fluid from his left elbow. The pt. was in a good mood for the rest of the day. Also, because of pt.’s history of arthritis, he had weakness in his joints which brought need for assistance. I partially assisted the pt. with feeding, however he was perfectly capable to eat independently.
