Migraine prophylaxis using Topiramate

Medical Practitioners have been using many different medications for the treatment and prevention of migraines. Some of these medications are effective in the treatment or prevention of migraines in some people but not on others. Topiramate has been a good option for migraine prevention. I am going to evaluate the effectiveness of topiramate in the prevention of migraines by looking at randomized control trials that have studied the effectiveness of topiramate by itself and one that compared topiramate and amitriptyline alone and in combination with for migraine prevention alongside the guideline.

This study had a total of 159 subjects on topiramate with a history of migraine as defined by the diagnostic criteria in the international classification of headache disorders. These subjects went through a pretreatment/washer phase of 70 days, including a baseline period (day -28 to day-1). They were then started on a single 25mg tablet of topiramate from day 1 to 7. The dose was then increased subsequently by 25mg each week up to a daily dosage of 100mg/day, given twice. The subjects dose was adjusted discretely based on tolerance to a minimum of 75mg/day. The study included adults between age 18 and 60 with a history of migraine greater than one year, 3 to 12 migraine episodes with pain greater than 5 on a scale, for 28 days in the last 3months. Twenty subjects were treated with topiramate. Baseline clinical characteristics were the same. The patients on topiramate were started on 25mg/day and titrated until tolerable dose with weekly increment s of 25mg up to a maximum of 200mg.
