A game involving a simulated helicopter doesn't seem controversial, but one of the game's designers was unhappy with work conditions and decided to take it out in the game's code. He created characters dressed in speedos that would walk around and kiss each other.
Number Seven: Tomb Raider.
Some code in Tomb Raider allowed players to turn the game into Nude Raider, which let them maneuver Lara Croft around in the nude. This game hack flies in the face of Croft's persona, which is of a strong, independent woman.

Number Five: Mario Party 8.
In one of the mini games featured in this family-friendly video game, one character exclaims, "Spastic!" This seems all good and fine until you take into consideration the fact that "spastic" is an offensive term in many other countries, and then the issues arise.
Number Four: Mind Quiz.
This game marketed itself as a mental trainer but was never very popular. However, what it became best-known for is its use of the word "spastic" in reference to the gamer. As was mentioned in the previous item, "spastic" is a derogatory term for sufferers of cerebral palsy, so there was much controversy surrounding this reference.
Number Three: LittleBig Planet.
This game seems super innocent, but you might be surprised. It was revealed that passages from the Koran were featured in the licensed track in one of the game's stages. The music had to be removed before the game was released.
