The Titanic sunken in the Pacific ocean, but who or what was responsible for the Titanic disaster?

It was a dark foggy night on Titanic, everything seemed normal but that night would go down in history. For starters it was a foggy on that night with almost no visibility says on page 18. The lookout could not see out. The lookout was also exhausted from a long hard day looking for icebergs or other threats to the ship. But that’s not the only thing that helped sink the Titanic.
