Tili Fisher Case Study

On 4/21/2017 Tili Fisher from Alternative Staff called Wellness Coach Teresa Cain on behalf of Richard Bellamy. Ms. Fisher stated that a referral for home health services were put in place. Ms. Fisher stated in order for Mr. Bellamy to receive services he must switch over to straight Medicaid. Ms. Fisher provided a documentation for Mr. Bellamy to sign and return. Wellness Coach Ms. Cain knocked on Mr. Bellamy residence door but there was no answer.
On 4/24/2017 per conversation with Wellness Coach Irram Nawaz stated she seen Mr. Bellamy in front of the building drinking his morning coffee. Wellness Coach Ms. Nawaz stated that Mr. Bellamy appeared to be alert and well-groomed. In conversing with Mr. Bellamy he had stated that after drinking

Bellamy came to the staff office appearing to be alert and groomed. Mr. Bellamy has been attending meeting with staff and becoming more active in attending SUS community meetings. Mr. Bellamy stated that he could not stay because he was on his way to get lunch at the dinner. Wellness Coach Ms. Cain asked when Mr. Bellamy return to stop in the staff office. The replied was “yes ma’am”.
At 1:20pm Mr. Bellamy returned expressing to staff that when he was on the street it was hard he caught a cold and had head lice. Wellness Coach Ms. Cain stated that it is good he returned and is attending his health services at the Manhattan VA. Mr. Bellamy said yes. Mr. Bellamy also signed documentation for Alternative Staff, INC to receive home health services. Mr. Bellamy new health insurance will fall under WELL CARE Manager Long Term Care. That will still provide Mr. Bellamy with dental, eye exams, primary care. In addition to being a Veteran Mr. Bellamy still will be able to attend the Manhattan VA because the WELL CARE health insurance will not affect those network providers.
Plan of Action: Wellness Coach Ms. Cain will follow up with Tili Fisher at Alternative Staff Inc. 718-972-2500 ext. 36 to receive a date of assessment with Maximus Inc. Stage of Change:
