In the hospital setting, I have came in contact with a Care Coordinator, but I never retained a full understanding of their job description and role in the health care team. In the future, it will remain critical to be aware of my resources and interdisciplinary team members, so I must seek out opportunities to interact and gain insight into other health care team member roles. The best way to get an understanding of another’s role is to place myself in it, so I am excited for the experience that tomorrow will bring. I will ask questions when I am unsure of the motive or purpose behind an action, question the RN about her role in patient care, and observe how the RN interacts not only with the patients, but also with other team members. When researching the job description of an RN Care Coordinator, I learned that the Care Coordinator assists the care team with setting goals for quality assurance and best practices. Job descriptions, specific to a Care Coordinator may include: assessing and screening patients, providing education about medical procedures, provide referrals, and create outcome goals and an overall care plan. If one desires to become a Care Coordinator, specific skills will assist, such as: compassion, respect for autonomy, critical thinking skills, team player, resourcefulness, leadership skills, and emotional stability. Serving as the bridge between patients and the healthcare system, Care Coordinators ensure that patients receive the care they deserve, by assisting in their understanding of medical conditions, medications, and treatment
