Polonius was hiding behind a curtain. Hamlet's entrance alarms Gertrude that she cries for help and Polonius echoes her cry and Hamlet thinking Polonius is Claudius, stabs Polonius to death. “O me, what hast thou done? Nay I know not. Is it the King?” (3.4. 30-32) Hamlet kills Polonius causing Ophelia become emotional distress resulting in her committing suicide. If Hamlet made the decision to kill Claudius when he had the chance then this would of not lead to his own death because it caused Hamlet and Laertes to fight. Hamlet is internally conflicted because he does not know what to do with Polonius’s body and causing Ophelia to commit suicide because of her father's death.
Regardless of all these examples of internal conflict, some events beyond Hamlet’s control. These actions of external conflict, however, did not lead to Hamlet’s demise. Despite the fact that Claudius has killed Hamlet's father and tries to kill Hamlet throughout the play. The greatest contributor to his death is his own inability to act. None of the rest of the action of the play would have occurred and Hamlet would not have died if Hamlet had been able to kill Claudius after speaking to his father, the ghost.
