Thelma is a 48-year old African American and Hispanic woman who has never been married. She identifies as a heterosexual female but struggles with her gender and identity. Thelma is an identical twin and has two older brothers and an older sister. Her father was absent all her life but had a stepfather, instead.

Winnicott, constructed this idea of the “holding environment”. It serves apart of psychotherapy to the parental relationship between a caregiver and infant, (Applegate 1997). The worker represents the caregiver and the client represents the infant. The concept is centered on providing a nurturing and safe environment for clients. This concept isn’t only about creating a safe environment for clients, but also, for yourself. Similar to Thelma’s treatment, we have naturally adopted the holding environment simply because it facilitates relationship building, support, and trust. As stated before, in treatment, Thelma engages completely different than she would if she engaged with her family. She is safe and secure in treatment, it is consistent and contingent. According to Beck (2011) the basic question to elicit automatic thoughts is "what was going through your mind"? This question will start a conversation to help identify and describe the problem or situation that which the problem stems from. This will help Thelma in achieving her treatment goal to address the sexual abuse she endured. Clients often do not recognize their automatic thoughts so by asking them to describe a specific situation helps to access them. Asking Thelma to relive the abuse through images may be difficult for her, but it’ll allow us to address her thoughts about feelings surrounding the abuse in that
