As a front line worker with people with developmental disabilities, I have noticed that in all the agencies I have worked, they are usually more careful when handling their clients’ medication administration issues. These agencies strive hard to ensure that clients are not overmedicated, missing medication doses and given wrong medication. The occurrence of any of these can lead to client’s death, deterioration in their health condition, as well as spelling harsh consequences for the staff who is responsible for the medication error. The agency’s license can be revoked if medication errors is rampant. Aside from that, it is also contrary to the agencies’ operational visions and values to not adhere to their avowed duty and standard of care for their clients whom they promised the Federal or provincial Government of Canada through Persons with Developmental …show more content…

At this point, according to Sower, Holland, Tiedke, and Freeman, 1957) as cited in Brad McKenzie and Brian Wharf (2010, p. 77) “a convergence of interest” has presented itself and as BradMckenzie and Brian Wharf (2010, p. 77) continue to argue, “a crisis of some magnitude has occurred and action will be initiated’. Working in the healthcare field always involves change. The demands of healthcare means nurses work in constantly changing environments; they must continually adapt to different needs, new technologies, government policies and other innovations (Bowers, 2011). As nurses we feel like change is imposed on us and our views and opinions are not taken into consideration during the decisions making process. The problem that was identified while working in the emergency room (ER) by our director was the low compliance with medication reconciliation when patients arrive at the ER. Medication reconciliation was developed to reduce medical mistakes and injuries through a process of creating and comparing a current medication list from the patient, information sources, and resolving discrepancies (Porcelli, Waitman, & Brown, 2010).…

The role and responsibilities of an RN especially with regards to medication administration, hold great significance as they coincide with legislations and codes. According to the discussed documents, Jo’s lack of competence was evident as demonstrated through her medication error, she had an inability to keep to her scope of practice and also to the medication management system created in the residential aged facility. Jo did not practice ethical nursing as the ethical issues that arose in this scenario were not dealt with correctly; she did not ask for help when she needed it, nor did she show respect to her colleague’s (the prescribers) role when she took on the role of the prescriber herself. Neither did she demonstrate the important values…

If I were the hospital administrator, chief of the medical staff or the chief of nursing, I would implement stringent standards to follow that provide maximum protection which would ensure that the administering of medication is performed safely and efficiently. The most common errors reported by healthcare providers, are those that have to do with medication errors. The fact that nurses are often front line providers who are required to administer medication to patients (at the direction of doctors), it is imperative that instructions be followed to the letter and practices and procedures carefully executed to avoid medication errors, serious injury or loss of life. Some of the following practices could be seen as causes of medication errors; failure to notate an order change, negligence with giving injections, failure to administer the appropriate medication, medications with similar sounding names, the wrong dosage, the failure to cease or discontinue medication and administering medication to the wrong patient. According to Showalter (2017), Negligence occurs when a person fails to live up to accepted standards of behavior.…

Administering medications to patients is a daily task performed by almost all nurses. In the Bachelor of Nursing curriculum, the 7 rights and 3 checks of medication administration is taught, practiced and performed by all nursing students to ensure proper habits are developed. The problem occurs when health care providers are no longer under supervision of an instructor, and short cuts are acquired. These short cuts, although time saving, ultimately cause more complications for the patient and the health care system. As a group, we have found that CARNA’s Medication Administration Guideline (2016) is not being implemented or enforced as it should to prevent medication errors.…
